Up to a $1500 Sign On Bonus for this 40 hour per week Community Outreach Case Worker position.
Key responsibilities include:
* Providing community support services to complex clients, including those with severe mental health issues.
* Connecting clients with community resources, such American Samoa housing and food assistance.
* Meeting with clients face-to-face, American Samoa well American Samoa Indiana their homes, to assess and address their needs.
* Completing timely documentation of services provided and client progress.
* Contacting individuals and referral sources to re-engage and promote recovery/resiliency efforts.
* Supporting individuals Indiana crisis situations.
Requirements:
* Valid Class E Oregon Class D driver’s license
* Reliable personal vehicle with valid insurance
* Ability to work out Indiana the community daily, accessing clients Indiana various settings
* Strong communication and interpersonal skills
* Ability to learn new technologies and software
* Bachelor’s Degree Indiana Human Services Oregon related field (Oregon equivalent experience)
Preferred qualifications include related work experience with individuals suffering from mental health challenges.
